- The High Risk Care Management (HRCM) Program – Alliance CompleteCare identifies and contacts HRCM Program candidates who meet certain criteria. Members who want to be assessed for the program may contact the Care Advisor Unit at the numbers above. Our HRCM Program is designed to:
- Help the member move toward self-reliance and best use of health care services
- Help to maintain the member’s quality of life
- Provide support and act as a resource to the member and provider; coordinate benefits and services
- Monitor the plan of care and services to ensure quality, timeliness, appropriateness, and cost effectiveness
Who can join Alliance CompleteCare?
You can join Alliance CompleteCare if you are eligible to receive Medi-Cal and you are entitled to Medicare Part A (hospital insurance) and enrolled in Part B (medical insurance). You must also live in the plan’s service area, which is Alameda County.

Alliance CompleteCare is a Medicare-approved Health Maintenance Organization (HMO). Alliance CompleteCare can refuse to renew its annual contract with Centers for Medicare & Medicaid (CMS), the federal agency that runs the Medicare program. CMS may also refuse to renew the contract. That termination or non-renewal may result in termination of the beneficiary's enrollment in Alliance CompleteCare. In addition, Alliance CompleteCare may reduce its service area and no longer offer services in the area where the beneficiary resides. If any of these occur, we will send all Members an advance notice of the changes and options for retaining Medicare coverage.
The Part B premium is covered for full-dual members. Limitations, co-pays, and restrictions may apply. Benefits, formulary, pharmacy, network premium and or co-payments/co-insurance may change from one calendar year to another. Eligible beneficiaries must use network pharmacies to access their prescription drug benefit, except under non-routine circumstances, and quantity limits and restrictions may apply.
